This form is to be used by members who are injured while in the performance of their duties. It is must be filed with the Retirement Board (not your employer) within 90 days of the event. It does not supplant any injury notices that are customarily required by your employer or Workers' Compensation Department. |

Reimbursement form to be used by accidental disability retirees for purposes of medical, hospital and prescription expenses |

Here’s everything you need to get started. |

|
You can get a refund for the amount in your annuity savings account if you terminate service without a pension. |
|
|
You can change your address, or the address of your beneficiaries. |
|
|
You can pick a beneficiary to receive a lifetime pension if you die before retiring. |
|
|
With this form, you can direct us to pay the amount you contributed to your account to your beneficiary in one lump sum if you die. |
|
|
You can request credit for your time spent in the armed forces. |
|
|
You may be able to buy credit for a previous public service job that you can put toward your pension account. |
|
| Referral report of potential fraud | If you know of a potential pension fund fraud, you can complete this form and send it to the state. |
