How to make record requests and inquiries

Only patients or those authorized by patients may request copies of records.

For questions about your bill

Boston EMS contracts with a company to manage transport billing. For questions related to your bill please call 844-353-7842. They are able to provide you with a copy of your bill, set up a payment plan, update insurance information, or assist with a hardship application.

To request a copy of your medical record

Patients can request copies of their transport record by calling Boston EMS at 617-343-2367. Select option 2, followed by option 2 again to speak to the Keeper of Records.

Gather necessary information

Ensure you have necessary patient information and incident details, including the date and time the patient was transported. You need to secure a patient authorization for release of medical records.

Make Request on ChartSwap

Go to ChartSwap. If you have not already done so, create an account.

Enter the necessary information to request a copy of a patient's bill or medical transport record and upload the patient authorization form.
